INTRODUCTION:

We are looking to recruit a Team Lead/CNM 2 for our Community Team,
based in Ballinasloe for Galway and the surrounding counties.

The Community Team Lead has a pivotal role in service planning,
co-ordinating, and managing activity and resources within Lauralynn's
community palliative care nursing services. The main responsibilities
are providing professional and clinical leadership, effective resource
management, staffing and staff development, and within a community
children’s palliative nursing care model.

The Community Team Lead/CNM 2 will work in collaboration with the
Assistant Director of Nursing (ADON) to support community palliative
nursing care service development ensuring continuity of care and
support for families and that adequate nursing resources are deployed
to the community.

ESSENTIAL CRITERIA - IDEALLY THE SUCCESSFUL CANDIDATE WILL POSSESS THE
FOLLOWING:

* 5 years post registration/qualification experience in Nursing.
* Experience of working with children or young people with
life-limiting conditions and their families
* Experience in delivering expert clinical practice
* Experience managing change and developing services
* Experience of working in a multi-disciplinary care team
environment where there is a commitment to working with the whole
family
* Experience of managing a complex case load
* Experience of managing and leading teams
* Full clean Driving License
